If your cover was cancelled as a result of non-payment of your premiums and you now want it back, you may be able to apply to reinstate your policy depending on your policy terms and conditions. Please refer to your Policy Document for the full terms and conditions that apply.

To start the process to request reinstatement of your cover, after cancellation you'll need to:

 Pay the premium
(no form or underwriting) 
Pay the premium and complete the form 
(with underwriting)
Lifetrack1-14 days     15 - 365 days  
Risk Protection Plan1-90 days91 - 180 days

To reinstate your cover

Find your policy number which is easily located on a previous annual statement.

Pay outstanding premiums, then (if you need to complete the form):

1.  Complete the relevant form for your product:
•    Lifetrack
•    Risk Protection Plan

It's important to be honest when completing these forms as it could have an impact if you need to make a claim.

2.   Send the completed form to:

All requests are subject to the underlying policy terms and conditions and Resolution Life reserves the right to accept or decline requests at its discretion. If your cover can be reinstated, we’ll confirm the terms and conditions of your request, including when cover will recommence.
